HAZELTON MUSICAL INSTRUMENT PROTECTION PLAN

Your new Hazelton instrument has been crafted with care and is covered by a two year limited factory warranty. The Hazelton Consumer Protection Plan is added protection to cover the cost of repairs for damage that are not covered by the warranty during the first year of ownership. This Consumer Protection Plan is extended to the original purchaser of the instrument for personal use only and is not transferable. Proof of original purchase is required to receive repairs of any kind under the plan. The Consumer Protection Plan is provided to you at no extra cost. There is a per-occurrence deductible charge only when you use the Plan. This deductible amount varies by instrument.

Coverage
The Hazelton Consumer Protection Plan covers repairs for any damage to your instrument that is not covered by the factory warranty except as noted below. This includes dents, breakage, stuck mouthpieces or slides, bent keys, missing or damaged pads. Return freight charges are also covered. Units will be returned freight prepaid by the same method as received. If your instrument cannot be repaired, it will be replaced with a new or refurbished instrument of the same quality. Hazelton will, at its sole discretion, make the decision to repair or replace the instrument.

Exclusions
The Hazelton Consumer Protection Plan does not cover loss due to any reason, instrument finish, normal wear and tear or maintenance. It does not cover consumables such as reeds, strings or lubricants. It does not apply to the instrument case.

The amount of the deductible charge varies with instrument per the following chart.

Using the Hazelton Consumer Protection Plan
If your instrument needs to be repaired, you must first obtain a Return Authorization number from Hazelton. To get this number, please call 1-800-592-9393, extension 118. You will be told how and where to send your instrument. You will need to provide proof of original purchase with the instrument when you return it. You can pay the deductible by credit card when you receive the Return Authorization number or you can enclose a check with the returned instrument. No work will be done until the deductible amount is received.

When your instrument is received at our repair facility, you will be contacted as to the disposition of the instrument. Every effort will be made to complete your repairs within 3 business days after receipt by Hazelton. The instrument will be returned to you using the same method of transportation you used to ship it. If you sent the instrument in by UPS ground, we will return it by UPS ground. If you need the instrument returned in different manner than it was sent, there may be additional charges.

All repairs are guaranteed for 90 days. If your instrument needs subsequent re-repair of the same problem, it will be done at no additional charge. If the instrument needs subsequent repair of new damage, a new deductible charge will be required. Hazelton will, at its sole discretion, make the decision as to whether or not any subsequent repairs are to be covered by the repair warranty. You must pay the freight to return the instrument to Hazelton and we will pay the freight back to you as with the original repair.

Please note in some cases, a minor, non-warranty repair may be performed by your authorized Hazelton dealer at a cost lower than the deductible. In this case, please have your dealer repair the instrument. This will not void your warranty. Use the Hazelton Consumer Protection Plan when your dealer’s estimate is more than the deductible amount.
